Home Care Needs Assessment Checklist

Determining the right level of home care starts with understanding what assistance is needed throughout a typical day. This checklist can help families organize important details before speaking with a home care provider.

1. Daily Care Needs

Write down the activities that require assistance, from getting up in the morning through bedtime. Include meals, personal care, household tasks, rest periods, appointments, and other regular activities.

2. Personal Care & Companionship

Consider how much hands-on assistance is needed with bathing, dressing, grooming, toileting, mobility, or other daily activities. Also note preferred hobbies, routines, and social activities if companionship is an important part of care.

3. Transportation

Determine whether assistance is needed for medical appointments, errands, or social activities. Discuss the appropriate transportation method with the provider, including whether caregiver transportation is available and what insurance or agency requirements apply.

4. Memory or Cognitive Changes

Note any memory loss, confusion, or changes in judgment that may affect daily safety. Share relevant diagnoses or concerns with the care provider so appropriate support and caregiver training can be considered.

5. Language & Communication

Identify any language, communication, hearing, or other needs that could affect how the client communicates with caregivers.

6. Hours of Care

Estimate how many hours of assistance are needed each day and when support is most important. A clear schedule helps the provider determine an appropriate starting care plan.

7. Dietary Needs

List food allergies, dietary restrictions, meal preferences, and cooking assistance that may be needed. Consider how groceries will be purchased or delivered when shopping assistance is required.

8. Specialized Care Needs

Tell the provider about any clinical or specialized tasks that require qualified professionals. Non-medical home care agencies may have limits on what caregivers can perform, so these needs should be discussed before services begin.

9. Medication Support

Prepare an up-to-date medication list and describe how medications are currently organized and monitored. Discuss any concerns about missed doses, reminders, side effects, or difficulties following the prescribed routine with the appropriate health care professional.

10. Care Coordination

Consider whether additional care coordination is needed to manage appointments, pharmacy needs, communication with family members, or other services. A clear division of responsibilities helps prevent important tasks from being overlooked.

Choosing a Home Care Provider

Before selecting an agency, families may also want to ask about:

  • State and local licensing requirements that apply to the agency
  • Caregiver screening and background checks
  • Caregiver training and ongoing supervision
  • Insurance coverage and worker protections
  • Written care plans and service agreements
  • How caregiver performance and client concerns are addressed
  • Backup coverage when a scheduled caregiver cannot work
  • After-hours or on-call availability
  • How families receive updates about care
  • The process for changing services as needs develop

The right questions can make the selection process clearer and help families understand how an agency manages care, communication, safety, and accountability.

When Transportation or Driving Becomes a Concern

Changes in vision, mobility, memory, judgment, or medication effects can sometimes affect a person’s ability to drive safely. If there are concerns, document specific observations—such as difficulty following traffic signals, unexplained vehicle damage, confusion while driving, or increased anxiety—and consider discussing them with the individual’s physician.

If driving needs to be reduced or stopped, plan alternative transportation in advance. This can make the transition more manageable while helping the individual maintain access to appointments, essential errands, and social activities.
